View, print, and/or order a new ID Card by logging into your UMR account.  Click to visit umr.com, then click on ID Card under My Menu.

Members of EyeMed do not need ID cards to receive services. Simply provide name and DOB.  You can print an ID Card by logging into your EyeMed account.
